运用3种方法检测Rh不规则抗体的临床应用

Clinical practice of irregular Rh antibody detection using three methods

目的:通过总结日常工作中不规则抗体检测结果,探讨凝聚胺法、抗人球蛋白法、微柱凝胶法3种特殊配血方法在检测Rh抗体方面的差别。方法:运用上述3种实验方法对不同效价的Rh抗体进行测定。结果:凝聚胺法、抗人球蛋白法、微柱凝胶法这3种方法均能检出血清中大部分不规则Rh抗体,但是3种方法存在差异。其中,凝聚胺法最为经济方便,微柱凝胶法最为敏感,而抗人球蛋白法最为可靠。结论:在配血过程中应采用多种方法检测有疑问的标本,尤其是对有妊娠史、近期输血史、短期内反复输血或以往输血有反应的患者,应采用多种方法检测,以避免输血反应的发生,确保临床输血安全;经典抗人球蛋白法仍是检出较弱抗体,排除非特异性凝集干扰的最好方法。

Objective： To study the differences among three kinds of experimental methods （polybrene, Coombs test, micro column gel）in detecting irregular antibody . Method： Rh antibodies with different titles were detected using these three methods. Result：These three methods all could detect most parts of irregular IgO antibodies, and polybrene method was most economical and convenient, micro column gel method was most sensitive, and Coombs test was most reliable. Conclusion： Adopting various methods to detect irregular antibodies would avoid the occurrence of blood transfusion reaction. Coombs test is the best way of detecting weaker antibody and precluding non--specific agglutination interference.
