乙型肝炎病毒cccDNA检测与血清学及病理诊断关系

目的建立和应用聚合酶链反应法(PCR),检测慢性乙型肝炎患者肝组织中乙型肝炎病毒(HBV)共价闭合环状DNA(cccDNA),探讨慢性乙型肝炎肝组织HBV cccDNA与乙型肝炎的血清学及病理关系。方法分别采用PCR法检测慢性乙型肝炎患者肝组织HBV cccDNA,同时用免疫组化法检测肝细胞中HBcAg、Pres1;荧光定量PCR法检测慢性乙型肝炎患者血清HBV DNA载量;电发光化学方法检测慢性乙型肝炎患者乙型肝炎病毒标志物。分析肝组织HBV cccDNA与血清HBV DNA载量、HBeAg、HBeAb,以及肝细胞内HBcAg、Pres1的关系。结果①127例慢性乙型肝炎患者肝组织HBV cccDNA与血清HBV DNA载量呈正相关(P<0.01)。②127例慢性乙型肝炎患者肝组织HBV cccDNA阳性组与HBV cccDNA阴性组血清HBV DNA载量及肝细胞内HBcAg、Pres1无显著性差异(P>0.05)。③HBV cccDNA阳性组的HBeAg检出率明显高于HBV cccDNA阴性组,HBV cccDNA阴性组的HBeAb检出率明显高于HBV cccDNA阳性组。结论测定肝组织中HBV cccDNA,对于了解乙肝患者病情和传染性强弱以及评价肝脏炎症活动度有重要意义。

Analysis of liver serum HBV cceDNA and HBV DNA load, HBeAg, HBeAb, liver cells HBeAg, Presl. [Results] 127 cases of patients with chronic hepatitis B and liver HBV eceDNA serum HBV DNA load was positively correlated （P 〈0.01）. 127 patients with chronic hepatitis B patients with HBV cceDNA in liver tissue HBV cecDNA positive group and negative serum HBV DNA load cells and liver HBcAg, Presl no significant difference （P 〉0.05）. HBV cccDNA group of HBeAg-positive rate was significantly higher than HBV eccDNA negative group, HBV cccDNA positive.HBeAb the negative group was significantly higher than the rate of HBV cccDNA positive. [ Conclusion] Measurement of liver HBV cccDNA, for understanding hepatitis B patients with infectious disease and inflammation of the liver as well as evaluating the strength of activity is of great significance.
