高血压患者的社区护理干预效果影响

The effect about community nursing intervention of patients with hypertension

目的:探讨社区护理干预对高血压患者缓解和改善病情的影响,并总结相关的社区护理经验。方法:调查了解社区内120例高血压患者对高血压的了解程度、饮食与运动、正确服药和血压控制等情况,制定针对性的护理干预措施。具体内容包括:提供健康教育指导、运动指导、饮食指导、心理护理、服药护理等。具体方法包括:发放资料、集中宣教、上门服务等。并与患者家属及时沟通,定期组织患者交流经验,构成患者家属,患者本人,患友,医护人员四位一体的多方位护理干预体系,从而促进患者病情的有效缓解和全面康复。结果:经过1年的社区护理干预,120例高血压患者的收缩压和舒张压显著下降。结论:社区护理干预能有效缓解或改善高血压患者的病情,提高患者的生活质量,是一种切实有效的方法。

Objective ：To discuss of impact of community nursing intervention on relieving and improving the illness of hypertension,and summarize related community nursing experience.Methods： 120 patients with hypertension community were surveyed about understanding of hypertension, diet and exercise, proper medication, control of blood pressure, and such conditions, specific nursing intervention measures were made, then. Specific content included： education of health, sports and diet, guidance of medication and psychological nursing care, etc. Specific approach include： providing related material, communication and education intensively, door to door service, etc. Families of patients were communicate timely and exchange of experience were organized regularly, so a four-in-one and many-side nursing intervention system which concluded families of patients, patients, patient-friends, nurse were build, to promote the fully recover and effectively relieving of patients. Results：Systolic blood pressure and diastolic pressure of 120 patients with hypertension decreased significantly. Conclusion：Community nursing intervention can effectively relieve hypertension or make the condition better, and improve the quality of life of the patients. So it is a kind of effective method.
