大肠癌致肠梗阻的外科治疗(附34例报告)

Surgical treatment of intestinal obstruction caused by colorectal carcinoma (34 cases)

目的探讨大肠癌致肠梗阻的外科治疗方法。方法回顾性分析该院2005～2008年34例大肠癌致肠梗阻病例的临床治疗资料。结果34例患者中,一期行右半结肠切除10例、横结肠切除1例、左半结肠切除5例、乙状结肠切除6例;Hartemman术式3例;肿瘤无法切除行短路手术2例、单纯造瘘术7例。结论一期切除吻合手术治疗大肠癌致肠梗阻是可行的,而合理地选择手术方式,正确的术中操作和围手术期处理对提高疗效,减少并发症十分重要。

[ Objective ] To study the surgical treatment of intestinal obstruction caused by colorectal carcinoma. [Methods] Retrospective analysis of the experience of surgical treatment for intestinal obstruction caused by coloreetal carcinoma in 34 patients from 2005 to 2008 was performed. [ Results ] Ten patients received one-stage right hemieolectomy, one patient received one-stage resection of transverse colon, five patients received one-stage left hemicolectomy, six patients received one-stage resection of sigmoid colon; three patients received hartemman. Anastomosis of ileum and tranaverse colon 2 cases, colon fistula 7 cases. [ Conclusion ] One stage resection and anastomosis is a feasible operation for intestinal obstruction caused by colorectal carcinoma. Appropriate choice of the operative procedure, skillful operative technique and prudential perioperative treatment are important for better results and lower complications.
