原发性肺淋巴上皮瘤样癌21例手术患者临床分析

The Clinical Analysis of 21 Patients with Lymphoepithelioma-like Carcinoma after Operation

背景与目的原发性肺淋巴上皮瘤样癌(lymphoe pithelioma-like carcinoma,LELC)是一种少见的肺部肿瘤,为肺大细胞癌的一个亚型。本研究回顾性分析LELC患者的临床特征、手术方法、实验室指标、术后放化疗及预后。方法回顾性分析胸科医院2004年-2008年间原发性肺淋巴上皮瘤样癌手术患者21例,对其临床资料及预后进行临床分析。结果21例LELC术后患者中行肺叶切除术15例,楔形切除术4例,全肺切除术1例,袖形切除术1例。术后接受化疗12例,接受放疗3例。电话随访至2009年4月31日为止,共4例患者死亡,中位生存时间(median surv ivaltime,MST)为49个月。结论LELC是一种罕见、独特类型的肺癌,经手术及术后辅助放化疗后有较好的预后。

Background and objective Lung lymphoepithelioma-like carcinoma is a rare subtype of large cell carcinomas of the lung.The aim of this study is to retrospectively analyze the clinical characteristics,surgical methods,laboratory inspection,chemotherapy,radiotherapy and prognosis of LELC.Methods From 2004 to 2008,clinical data were collected from 21 patients who were treated in Shanghai Chest Hospital.The correlation between clinicopathological characteristics and prognosis was evaluated.Results Of the 21 patients,15 patients had lobectomy;4 patients had wedge resection;1 patient had pneumonectomy and 1 patient had sleeve resection.12 patients received chemotherapy and 3 patients received radiotherapy after operation.Until 2009-4-31,4 patients died,and the median survival time（MST） was 49 months.Conclusion Lymphoepithelioma-like carcinoma of the lung is a very rare and unique subtype of large cell carcinomas of the lung,which has a better prognosis with surgical,chemotherapy and radiotherapy.
