缺血性心肌病不同部位室壁运动异常与心率震荡的关系及比索洛尔的干预效应 被引量:1

摘要 目的研究缺血性心肌病不同部位室壁运动异常与心率震荡现象的关系,以及比索洛尔对其的影响。方法86例缺血性心肌病患者按照心脏彩色多普勒显像将节段性室壁运动异常特点分为下后壁运动异常组(A组,n=42)和前壁运动异常组(B组,n=44),以年龄相当的40例健康体检者作为对照组,再根据是否服用比索洛尔,将A、B组缺血性心肌病患者设立基础治疗组和比索洛尔治疗组,利用24h动态心电图分析系统获得所有研究对象的室性期前收缩情况,分别计算对照组和缺血性心肌病患者治疗前及治疗后8周不同亚组的震荡初始(TO)和震荡斜率(TS)2个指标。结果缺血性心肌病患者均存在TO及,TS异常。TO值B组(0.53±0.36)%〉A组(0.48±0.44)%〉对照组(-0.35±0.56)%;TS值B组(11.22±0.71)ms/RR〈A组(14.08±Oj52)ms/RR〈对照组(24.16±1.32)ms/RR(P均〈0.05)。与治疗前比较。治疗8周后A、B组中基础治疗纽知比索洛尔组T0值均显著降低,Ts值均显著升高(P均〈0.01),但比索洛尔治疗组TO值较基础治疗组降低更显著,TS值升高更明显(P均〈0.05),B组中比索洛尔组治疗前后TO及TS差值较A组高[ΔTO值:A组(-0.50±0.69)%与B组(-0.64±0.98)%,△Ts值:A组(4.28±2.58)ms/RR与B组(7.39±2.63)ms/RR,P均〈0.05]。结论缺血性心肌病患者心率震荡现象与其节段性室壁运动异常不同部位有关,前壁运动异常患者较下后壁运动异常患者心率震荡减弱或消失现象严重;比索洛尔可有效改善缺血性心肌病患者的心率震荡现象,对前壁运动异常患者的心率震荡影响尤为明显。
