初诊糖尿病患者强化治疗卡在门诊复诊中的应用

Research of intensive treatment card in newly diagnosed diabetes patients during the out - patient referral

目的探讨初诊糖尿病患者强化治疗卡在门诊复诊中的应用及效果。方法对2008年4月1日开始在我科住院治疗的初诊2型糖尿病患者中实施强化治疗卡。患者在出院时由责任护士发放自行设计的强化治疗卡,并协助患者在门诊复诊中应用。观察患者1年后遵医率、低血糖发生率及糖化血红蛋白控制达标率的变化,并与实施治疗卡前比较。结果实施后患者遵医率、糖化血红蛋白控制达标率明显提高,而低血糖发生率明显降低(均为P<0.01)。结论糖尿病强化治疗卡方便患者门诊复诊,可以提高患者的遵医率。

Objective To explore the effect of intensive treatment card in newly diagnosed diabetes patients during the out - patient referral. Methods Forty patients were enrolled. They got a self - designed intensive treatment card when they were discharged from hospitals and were asked to use the card during their out - patient referral under the assistance of charge nurses. After 1 year, the rate of therapy compliance, the incidence of hypoglycemia and the rate of HbA1 c control were collected. And to and implementation of the treatment cards of patients before the comparison. Results After the application of intensive treatment cards, the rates of therapy compliance and HbA1 e control were significantly higher, and the incidence of hypoglycemia was significantly lower than that before （P 〈 0.01 ）. Conclusion The application of intensive treatment cards in diabetes outpatients is beneficial to increase their therapy complance.
