摘要
目的探讨肝癌脑转移病例的临床特点和治疗方法。方法回顾性分析1994年1月～2008年6月我院收治原发性肝癌发生脑转移患者的临床特点、治疗方法及生存期。结果本组共8例肝癌脑转移病例,其中7例为男性,中位发病年龄为50.0岁。5例患者有慢性乙型肝炎、肝硬化病史。患者血中甲胎蛋白含量7例>1000ng/ml,并且2例>10000ng/ml。头痛是最常见的首发症状(4/8),3例患者出现头晕、轻偏瘫、感觉障碍,2例出现复视、意识障碍,1例出现构音障碍,6例患者同时伴有至少2种症状,5例患者同时伴有肺转移,4例患者在2个月内死亡。结论肝癌脑转移患者预后很差,其治疗模式有待积极探索。
Objective To investigate the clinical characteristics and treatment methods of hepatocel lular carcinoma （HCC） patients with intracranial metastasis. Methods From January 1994 to June 2008, 8 HCC patients with intracranial metastasis were treated in our hospital. Clinical characteristics, treatment methods, and surrival time were analyzed retrospectively. Results Of the 8 patients, there were 7 males. The median age was 50. 0 yeaes. Five patients have chronic hepatitis B and cirrhosis. Alpha fetoprotein 〉 1000ng/ml was noted in 7 patients; extraordinary high level （ 〉 10000ng/ml） was also seen in 2 patients. Headache as the initial manifestation was common in our series （4/8）. Hemiparesis, dizzy, or sensory disturbance was found in 3 cases, diplopia in 2 cases, conscious disturbance in 1 case and dysarthria in 1 case. Six patients had two or more symptoms. Five patients experienced pulmonary metastasis. Four patients died within two months. Conclusion The prognosis of HCC with intracranial metastasis is poor, and its treatment requires further research.
