糖尿病视网膜病变辨证分型及证候要素分布的文献研究

Literature Research about Distribution of Typing of Syndrome Differentiation and Syndrome Factors of Diabetic Retinopathy

目的通过对糖尿病视网膜病变现代文献系统评价,研究其中医证型及证候要素的分布规律,为进行该病证候的规范化及客观化研究提供依据。方法在设定的检索策略及标准下纳入合格文献,规范证名、提取证侯要素,利用Epida-ta2.0软件建立文献数据库,使用SPSS13.0统计软件对其证型及证候要素频数进行统计分析。结果糖尿病视网膜病变的常见证型为气阴两虚、阴阳两虚、阴虚内热、血瘀、气滞血瘀、阴虚火旺、肝气郁结、肝肾阴虚等;病位主要涉及肝、肾、脾;病性以阴虚、气虚、血瘀、火(热)、阳虚为主。结论目前糖尿病视网膜病变的中医证候名称及诊断标准不统一,不便于临床应用及学术交流,有必要引入"证侯要素"的概念,为实现证候要素量化诊断及证候要素应证组合奠定基础。

Objective To research the distribution discipline of typing of syndrome differentiation and syndrome factors on Diabetic Retinopathy （DR） by system evaluating the neoteric literature of DR, and to give some evidences on the research of standardization and objectivity about TCM syndrome. Methods According to retrieval strategy and inclusion criteria collect neoteric literature, to standardize the title of syndrome and extract syndrome factors, build up a literature data - base by epidata2.0 software and analyze the data by SPSS13.0 software. Results The connnon types of syndrome are deficiency of ＂Qi＂ and ＂ Yin＂ , deficiency of ＂ Yin＂ and ＂ Yang＂, interior heat caused by ＂ Yin＂ deficiency, blood stasis, ＂Qi＂ stagnation and blood stasis, excessive pyrexia caused by ＂ Yin＂ deficiency, depression of liver -＂ Qi＂ , ＂ Yin＂ -deficiency of liver and kidney . etc. The principal positions of the illness are liver, kidney and spleen. The principal characteristics of the illness are ＂ Yin＂ deficiency, ＂ Qi＂ deficiency, blood stasis, fire （heat）, ＂ Yang＂ deficiency. Conclusion Now the title of syndrome and the standard of diagnosis of syndrome are so complicated that hard to be used in clinical application and academic communication. So lead in the new concept of syndrome factors will establish the foundation for implementing quantization diagnosis about syndrome factors and the combination of syndrome factors.
