复方醋酸棉酚片治疗子宫肌瘤83例临床疗效观察

Observation of the effect of compound gossypol acetate tablets in the treatment of uterine leiomyoma

目的观察复方醋酸棉酚片治疗子宫肌瘤的临床疗效。方法将83例子宫肌瘤患者随机分为两组。A组42例,口服复方醋酸棉酚片20mg/d;B组41例,口服米非司酮15mg/d。均从月经周期第3天开始服药,连服3个月。治疗前后用B超测量子宫及肌瘤体积;比较治疗前后子宫动脉及肌瘤血管阻力指数,同时检测促卵泡素、黄体生成素、泌乳素、孕激素、雄激素指标变化情况。结果两组患者在治疗期间均出现闭经,临床症状均有明显改善。两组子宫及肌瘤体积均明显缩小,A组子宫体积平均缩小58.55%,差异有统计学意义(P<0.01);子宫肌瘤平均缩小52.2%,差异有统计学意义(P<0.01);B组子宫体积平均缩小53.97%,差异有统计学意义(P<0.01);子宫肌瘤平均缩小47.81%,两组之间差异无统计学意义(P>0.05)。两组患者子宫动脉及肌瘤血管阻力指数均有增加,差异有统计学意义(P<0.05);两组治疗前后激素水平改变差异有统计学意义(P<0.05),但两组间治疗前后比较差异无统计学意义(P>0.05)。结论复方醋酸棉酚片是一种治疗子宫肌瘤的安全有效药物,口服复方醋酸棉酚片治疗子宫肌瘤是一种可行的保守治疗方法。

Objective To evaluate the effect of treating uterine leiomyoma with Compound Gosspol Acetate tablets. Methods 83 patients with uterine leiomyoma were divided into two groups randomly,among which 42 were treated with 20 mg of Gossypol Acetate tablets daily and 41 with 15 mg of rnifepristone daily for 3 months, starting on the third day of the menstrual cycle. Volume changes of leiomyoma and uterus were measured with ultrasonography before the treatment and after three months of therapy,and comparasion was made about the level of serum re- productive hormone and blood flow of uterus. Results The clinical symptoms were improved obviously after treat ment. The size of uterus and myoma is decreased obviously. After 3 months of treatment, the uterine volume decreased by 58.55% and the teiomyoma volume decreased by 52.2% in group A. Compared with group B（53.9% and 58.55% respectively） ,there was no significant difference in the two treatment groups（P〉0.05）. Resistant index of artery of uterus and myoma were increased in both two groups（P〉0.05）. The level of serum reproductive hormone, stimulating hormone, luteinizing hormone, lactation hormone, estrogen, progestogen hormone and rosterone hormone were reduced obviously. Although there were significant difference in each group before and after therapy （P〈0.05） ,there was no significant difference in the two treatment groups（P〉0.05）. Conclusion Compound Gossypol Acetate tablets is a practical and helpful drug in treating uterine leiomyoma,Take orally compound Gossypol Acetate tablets is an available conservative treatment of uterine leiomyoma.
