妊娠期交通事故伤的急诊救治

Emergency treatment of traffic accident in the pregnant patients

目的探讨妊娠期交通事故伤的急诊救治方法。方法回顾性分析2000—2007年收治的17例妊娠期交通事故伤的临床资料。结果本组17例受伤孕妇中，孕妇死亡2例，死胎2例，流产（引产）3例，2孕妇出院后胎儿流产，余胎儿存活。结论交通事故伤成为孕妇非产科死亡的主要原因，急诊救治必须建立在充分了解患者妊娠期生理变化的基础上，首要的救护努力应聚焦在孕妇身上，孕妇稳定的生命体征是胎儿存活的保证。放射线检查与超声检查有时是必要的。必须坚持多学科协作，迅速组织急诊医师、产科医师、麻醉师、放射医师、相关外科医师和急诊护士组成救治团队参与抢救。

Objective To study the emergency treatment of traffic accident in the pregnant patients. Methods 17 pregnant patients with traffic accident were treated in our hospital from 2000 to 2007. Their clinical data were reviewed retrospectively. Results Of all pregnant patients, 15 cases were cured and two died, two fetuses died and three were aborted, other two fetuses were spontaneous abortion. Conclusion Traffic accident is the primary cause of nonobstetric - related death during pregnancy. Numerous changes of the pregnant female＇s physiology must be considered. Initial resuscitation efforts should focus on the pregnant patient because stable vital signs provide the best chance for fetal survival. Radiographs as well as abdominal ultrasound are useful. No specific fetal abnormalities have close links with standard low - intensity magnetic resonancg imagine. An experienced muhidisciplinary team consisting of an emergency physician, obstetrician, orthopaedic surgeon, anesthesiologist,radiologist and nursing staff will optimize the treatment of both pregnant patient and her fetus.
