Steri-Oss种植体支持固定和可摘义齿修复牙列缺失患者的临床回顾研究

The clinic retrospective study of Steri-Oss implant-supporting fixed and removable prostheses in edentulous patients

目的:研究十年观察期内用Steri-Oss种植体修复牙列缺失患者种植体生存率和影响因素。方法:自1995-2006年,共有57例无牙颌患者接受种植与修复治疗,共353颗Steri-Oss种植体。其中81颗种植体(22.95%)做固定修复,272颗种植体(77.05%)做可摘义齿修复,观察性别、种植体部位和修复类型对种植体成功率的影响。结果:其中有9例病人24颗种植体脱落,种植体成功率93.20%,女性病人的种植体生存率略低,但性别没有显著的差异(P>0.05)。累计的种植体生存率上颌骨92.64%,略低于下颌骨93.68%,但统计学上的没有显著差异(P>0.05)。种植体支持的固定修复的十年成功率是97.5%,种植体固位的可摘义齿成功率99.02%,累计的种植体生存率在可摘与固定义齿修复之间没有显著性差异(P>0.05)。结论:确认了种植体支持无牙颌病人修复学中长期成功率。种植体固位的可摘全口义齿治疗方式在种植体生存率上和种植体支持的固定义齿的结果是相同的。

Objective： The purpose of this retrospective study was to investigate the fixed and removable Steri-Oss implant-supported prostheses in fully edentulous patients within a functional period and to evaluate the implant survival rates with reference to gender, jaw and type of prostheses. Methods： A total of 353 Steri-Oss implants were placed in the edentulous maxilla from 57 patients, mandible or both jaws to allow a prosthetic rehabilitation with fixed [81 implants （22.95% ）] or removable [272 implants （77.05% ）] implant-retained prostheses. The survival rate of the implants was evaluated according to gender, implant location and type of prostheses that were used in this study, and a comparison of the parameters was performed to identify possible success factors. Result： 9 patients lost a total of 24 implants, which translates into a cumulative implant survival rate in all the patients of 93.20%.The study shows a slightly lower survival rate for females but there was no significant difference between the genders. The cumulative implant survival rate in maxilla （92.64%） was little lower than in the mandible （93.68%）, but the observed differences were not statistically significant（P0.05）. The 10-year cumulative survival rate of implants after rehabilitation that support a fixed restoration was 97.50% and a removable implant-retained overdenture was 99.02% . The cumulative implant survival rates showed no significantly difference between the two type of prosthetic rehabilitation （P0.05）.Conclusion： This retrospective study confirms good clinical outcome of implant-supported treatment concepts for the rehabilitation of totally edentulous patients in a medium-term perspective and that the implant survival rate is not correlated with gender and implant location and the type of prostheses with the Steri-Oss implant system. These preliminary findings also show a successful application of Steri-Oss implants to support two types of prosthetic rehabilitation.
