动态静脉尿路造影在泌尿系统中的诊断价值

Research on dynamic X ray intravenous urography

目的探讨动态静脉尿路造影(DIVU)在泌尿系疾病诊断中的临床价值。方法动态法静脉尿路造影558例,对其检查方法、X线表现进行研究,且取传统造影法(IVU)500例就其中X线诊断与临床病理诊断符合率,逆行尿路插管造影率进行比较分析。结果动态法静脉尿路X线诊断与临床病理诊断符合率为91.4%,传统法符合率为84.6%,两者差异有显著性。动态法造影558例中行逆行插管造影96例,占17.2%,传统法造影500例中行逆行插管造影134例,占26.8%,两者差异有显著性。DIVU方便快捷,不受年龄、体质及腹部病变的限制,拓宽了尿路疾病检查的适应症。结论动态法静脉尿路造影能提高尿路病变的X线诊断准确性,减少逆行插管造影。对尿路梗阻性病变、泌尿系肿瘤、先天畸形、外伤等,能作出准确定位与定性诊断。

Objective To find out the clinical value of dynamic intravenous urography （DIVU） in the diagnosis of urologic diseases and improve the accuracy of X-ray diagnosis. Methods A research of 558 DIVU cases was conducted regarding their ways of examination and X-ray presentation, which was compared with 500 cases of conventional intravenous urography （IVU） in their consistency of X-ray diagnosis with clinical pathological diagnosis. Results The consistency rate of X-ray diagnosis with clinical pathological diagnosis in DIVU was 91.4%, which was different from 84.6% of that in IVU （P〈0.01）. The rates of retrocatheterism in DIVU and IVU were 17.2% and 26.8%, respectively （P〈 0.01）. DIVU could be easily applied and did not limit age, health and ventral pathological changes. It also provides a more versatile way of examination of urologic diseases. Conclusion DIVU can improve the accuracy of X-ray diagnosis and reduce retrocatheterism.
