自体血回输在妇科腹腔内出血手术中的应用

Application of autologous blood transfusion in gynacological patients with acute intraabdominal hemorruage

目的探讨自体血回输在妇科急性腹腔内出血手术中的安全性和有效性。方法对35例腹腔内出血患者实施了自体血回输,其中,异位妊娠输卵管破裂30例,卵巢破裂5例,全部病例经急症手术和病理证实。对患者资料进行回顾性分析。结果患者伴不同程度失血性休克,均经腹腔穿刺证实腹腔内有游离不凝血。35例患者均采用了自体血回输,回输自体血总量达31 950 ml,平均每例(800±227)ml。无一例发生严重并发症,所有患者恢复良好。结论妇科急性腹腔内出血手术中回输合格的自体血安全有效,既可节约血源,又可避免异体血输入引起的多种疾病传播等不良反应。

Objective To investigate the safe and efficacy of autologous blood transfusion in gynaecological patients with acute intraabdominal hemorrhage. Methods A retrospective analysis was made in 35 gynacological cases with acute intraabdominal hemorrhage, of whom 30 cases suffered from heterotopic pregnancy and 5 cases were with rupture of the ovarium. All cases were diagnosed by operation and pathology. Results All patients showed hemorrhagic shock in different degrees and uncoagulatcd blood in the abdomen was confermed by abdominal puncture. Autologous blood transfusion[averaged （800±227） ml/case] was performed in 35 cases. All patients recovered with no serious complications. Conclusion Qualified autologous blood transfusion is safe and effective in emergent surgery for gynaecological intraabdominal hemorrhage with the advantages of saving bank bloob and avoiding the spread of diseases.
