臂丛神经根性损伤术前术后的康复治疗

Pre and post operative rehabilitation after root injuries of brachial plexus

目的介绍臂丛神经根性损伤术前、术后进行综合性康复治疗的方法与效果。方法选择１６例臂丛神经根性损伤，手术均行腓肠神经桥接于患侧膈神经和肌皮神经之间，副神经移接于肩胛上神经。随机分为康复组和对照组。康复组在术前、后均作系统性、综合性的康复治疗；而对照组则不行康复治疗。手术后６个月时对肌皮神经、肩胛上神经作肌电图检测。结果康复组：８例中肩胛上神经有再生电位者达１００％，肌皮神经有再生电位者为５０％。对照组：８例中肩胛上神经有再生电位者占６２．５％，肌皮神经有再生电位者为１２．５％。结论康复组的疗效明显优于对照组，对臂丛神经根性损伤患者术前。

Abstract Objective To introduce the method and evaluate the effect of pre and post operative rehabilitation for root injuries of brachial plexus. Methods 16 cases of root injuries of brachial plexus, in which phrenic nerve transfer to the musculocutaneous nerve with sural nerve grafting and accessory nerve transfer to the suprascapular nerve were done, and were randomised into experimental group and control group.In experimental group,patients received systematic pre and post operative rehabilitation,whereas in control group ,patients were given no rehabilitation treatment. Electromyographic examination of suprascapular nerve and musculocutaneous nerve was carried out 6 months later post operatively. Results In rehabilitation group, nascent potentials of suprascapular nerve and musculocutaneous nerve were demonstrated in 8 cases(100%) and 4 cases (50%) respectively, while in control group, the percent was 62.5% and 12.5% respectively. Conclusion Pre and Post operative rehabilitation is an effective and necessary treatment for root injuries of brachial plexus.
