重度肝外伤的诊断与治疗

DIAGNOSIS AND TREATMENT OF SEVERE LIVER TRAUMA

目的总结重度肝外伤临床诊断与治疗的经验,进一步提高诊治疗水平。方法回顾性分析32例重度肝外伤病人的诊治资料。结果32例均行手术治疗。治愈28例(87.5%),死亡4例(12.5%)。死亡原因:严重失血性体克1例,合并严重脑挫伤1例,术后合并多器官功能衰竭2例。结论准确进行伤情评估,早期诊断和合理的术式是重度肝外伤救治成功的关键;重度肝外伤常需联合应用几种术式才能获得良好效果。

Objective To summarize the experience in diagnosis and treatment of severe liver rtauma and improve the diagmosis and therapeutic level. Methods Diagnosis and treatment of 32 patients with severe liver were retrospectively reviewed. Results All of the patients were treated by operation. The curative rate was 87.5% and mortality wasl2. 5%. The causes of death were hemorrhagic shock in 1 patient, severe cerebral contusion in 1 patient,multiple organs failure in 2 patients. Conclusion Aceursate evaluation of injury, recognition and proper opertive manners arc the key factors of successful rescue of severe liver trauma. For taking a good efficacy, a combined operative manners is oftennecessary in the severe liver trauma.
