后腹腔镜肾囊肿去顶术治疗单纯性肾囊肿

Retroperitoneoscopic unroofing of simple renal cyst

目的总结后腹腔镜肾囊肿去顶术治疗单纯性肾囊肿的临床经验,讨论人工后腹腔建立的方法。方法1999年12月～2008年12月,78例后腹腔镜肾囊肿去顶术治疗单纯性肾囊肿。右侧肾囊肿47例,左侧肾囊肿31例,肾囊肿位于肾上极者21例,肾下极者47例,肾中段者10例。采用开放式和闭合式方法进行人工后腹腔建立。结果75例手术成功,3例术中转开放性手术。手术时间50～100 min,平均86.6 min。53例开放式方法人工建立后腹腔,25例闭合式方法人工建立后腹腔。结论后腹腔镜肾囊肿去顶术治疗单纯性肾囊肿疗效可靠,手术创伤小,术后恢复快。开放式人工后腹腔建立更易掌握、更加安全。

[ Objective ] To summarize the experience in retroperitoneoscopic unroofing of simple renal cyst, and to discuss the means of man-made retroperitoneum. [Methods] From December 1999 to December 2008, 78 patients with simple renal cyst underwent retroperitoneoscopic unroofing in the hospital. Forty-seven cysts were on the right side and thirty-one on the left. Twenty-one renal cysts were situated in the upper renal pole, forty-seven in the lower renal pole and ten in the middle part of the kidney. Man-made retroperitoneum was established by open or close means. [ Results ] Operations on 75 cases were successful, 3 cases converted to open surgery. The operation time was 50-100 min with the mean of 86.6 min. Man-made retroperitoneum was performed in 53 cases by open manner and in 25 cases by close manner. [ Conclusions ] Retroperitoneoscopie unroofing of simple renal cyst is an effective and minimally-invasive procedure. Patients recover quickly after operation. Man-made retroperitoneum by open manner is easy and safe.
