三联疗法治疗儿童幽门螺杆菌感染(附46例临床观察)

目的:探讨含质子泵抑制剂的三联疗法(奥美拉唑、阿莫西林、奥硝唑三联)治疗儿童幽门螺杆菌(HP)感染的疗效及安全性。方法:经13C-尿素呼吸试验(13C-UBT),证实为HP感染,反复腹痛儿童68例,随机分为治疗组46例,予以奥美拉唑0.8 mg/(kg.d)1次/d,阿莫西林40 mg/(kg.d)分3次口服,奥硝唑10 mg/(kg.次),2次/d,三联治疗;对照组22例,予以雷尼替丁4 mg/(kg.次),2次/d。其余抗生素与治疗组相同,疗程10 d。停药后4周门诊复查13C-UBT。结果:两组儿童腹痛均有缓解,缓解率100%;治疗组HP转阴45例,根除率97.8%;对照组转阴18例,根除率81.8%,两组幽门螺杆菌根除率有显著差异(P<0.05)。结论:奥美拉唑,阿莫西林、奥硝唑三联治疗儿童HP感染临床疗效好,HP根除率高,未见明显不良反应。

Objective：The objective is to discuss the curative effect and safety of combination treating method（Omeprazole,Amoxicillin and Ornidazole） of proton pump inhibitors in treating Hp.Methods：Through 13C-UBT,Hp was confirmed.Sixty-eight children suffered from abdominal pains.They were randomly divided into treatment group with 46 cases,treated by 0.8mg/（kg·d） Qd of Omeprazole,40mg/（kg·d） of Amoxicillin for three times and 10mg（twice a day） of Ornidazole.In the control group,there were 22 cases treated by 4mg/Bid.Other Antibiotics were the same with those of the control group, curative peiord being 10d. Four weeks after the medicine treatment, 13C -UBT was checked once more. Results：Abdominal pains in both groups were better. The relieving rate was 100%. Forty -five turned into negative regarding HP in the control group and the curative rate was 97.8%. Negative transference number in the control group was 18 with the curative rate of S1.8%, with remarkable difference between the two （P 〈0. 05）. Conclusiotm ： This combination treatment approach presents good curative effect in treating children ＇ s HP infection and HP was cured at a high rate without obvious side effects.
