BiPAP无创呼吸机治疗COPD合并Ⅱ型呼吸衰竭24例疗效观察

The therapeutic effect of BiPAP non-invasive ventilator on COPD with typeⅡrespiratory failure:24 cases report

目的探讨BiPAP无创正压通气治疗慢性阻塞性肺疾病(COPD)并发Ⅱ型呼吸衰竭的疗效。方法24例COPD并发Ⅱ型呼吸衰竭患者随机分为治疗组和对照组各12例,对照组给予抗感染、通畅气道、持续低流量吸氧、改善通气等常规治疗。治疗组除常规治疗外加用BiPAP呼吸机辅助通气治疗。结果治疗组患者临床疗效及心率、呼吸频率、血气分析等指标的改善优于对照组,差异有统计学意义(P<0.05)。结论无创双水平气道正压机械通气辅助治疗COPD合并Ⅱ型呼吸衰竭具有肯定的疗效。

Objective To evaluate the clinical effect of bilevel positive airway pressue （BiPAP） ventilation in treatment COPD with type Ⅱ respiratory failure. Methods 24 cases with COPD and type Ⅱ respiratory failure were divided into treatment group （ n = 12） and control group（ n =12）. The control group was given anti-infection,clean airway, continuously inhaling low concentration oxygen with ventilation improvement. The treatment group was treated with BiPAP ventilation in addition to the regular therapy. Results The heart rate, respiratory frequency and arterial gas analysis in treatment group were improved significantly than that in control group （ P 〈 0.05 ）. Conclusion It suggested that the non-invasive BiPAP assisted ventilation was eertainly effective to COPD with type II respiratory failure.
