伴有抑郁障碍的恶性血液病患者抗抑郁治疗前后细胞免疫及细胞因子的变化

The change of cellular immunity,interleukin-2 and interleukin-6 after antidepressive treatment in malignant hematogenesis pantients with depressive disorder

目的探索抗抑郁治疗对伴有抑郁的恶性血液病患者细胞免疫功能及细胞因子白介素-2(IL-2)、白介素-6(IL-6)的影响。方法将汉密尔顿抑郁量表(HAMD)评分≥20分的49例伴有抑郁的恶性血液病患者随机分为干预组和对照组,干预组给于抗抑郁药治疗,两组分别于入组前及第四周末测定T细胞亚群和血浆IL-2、IL-6,并分别于第2周末及第4周末再次进行HAMD评定。结果(1)干预组HAMD评分显著下降(P<0.01),对照组无明显变化;(2)干预组治疗后T细胞亚群中CD3、CD4及CD4/CD8显著高于治疗前(P<0.05),CD8无明显变化,对照组治疗前后T细胞亚群中均无明显变化;(3)干预组治疗后血浆IL-2、IL-6均较治疗前显著下降(P<0.05),对照组两者未发生明显变化。结论抗抑郁药既能有效改善恶性血液病患者伴随的抑郁症状,又能改善患者的免疫功能。

Objective To explore the effect of antidepressive treatment on cellulr immunity , interleukin -2 （IL -2） and interleukin- 6 （IL- 6） in malignant hematogenesis patients accompanied with depressive disorder. Methods A total of 49 malignant hematogenesis pantients with depressive disorder whose sclore of HAMD was over 20 were randomly divided into experiment groupantidep ressive drugs treated with and control group. T cell subset and plasma IL -2 and IL -6 levels were detected before treatment and at the end of 4th week after treatment. The severity was evaluated with HAMD before and at the end of 2nd and 4th after treatment. Results 1. The score of HAMD in experiment group decreased significantly after treatment （P 〈0.01）, but there had no significant afrer treatment change in control group（P 〉0.05） ;2. CD3,CD4 and CD4/CD8 in experiment group significantly increased（P 〈0.05）,CD8 didn＇t significantly change, while all of CD3 ,CD4,CD8 and CD4/ CD8 hadno significant change in control group （P 〉 0. 05）. 3. IL- 2 and IL- 6 plasma levels significandy decreased after treatment in experimet group（P 〈0.05） ,but there had no significant change in control group. Conclusion Antidepressive drugs could improve depressive symptoms and elevate immunity function in patients with malignant hematogenesis
