摘要
目的:检测碱性成纤维细胞生长因子(bFGF)在心房颤动(房颤)患者心房组织中的基因表达,探讨其与心房纤维化的关系。方法:75例风湿性心脏瓣膜病(风心病)接受换瓣手术者被分为三组:窦性心律组(34例),阵发性房颤组(11例),慢性房颤组(30例);于术中切取右心耳组织,分别采用半定量逆转录聚合酶链反应和免疫组化技术测定bFGFmRNA含量和蛋白含量。结果:与窦性心律组比较,bFGF的mRNA和蛋白在阵发性房颤组和慢性房颤组的表达均显著上调(P均<0.001),而且慢性房颤组的较阵发性房颤组的明显上调(P<0.05)。同时,bFGF的mRNA和蛋白水平均与房颤持续时间(分别为r=0.330,P=0.005和r=0.299,P=0.010)及左心房内径呈正相关(分别为r=0.342,P=0.003和r=0.285,P=0.015)。结论:心房组织中bFGF的基因表达上调可能参与房颤的发生和维持,与房颤的心房纤维化密切相关。
Objective: To investigate the gene expression of basic fibroblast growth factor (bFGF) in patients with atrial fibrillation (AF). Methods: The right atrial tissue samples were taken from 75 patients with rheumatic heart disease who underwent heart valve replacement surgery. In which 34 patients were in sinus rhythm, 11 patients had paroxysmal AF and 30 patients had chronic AF. The mRNA and protein level of bFGF was measured by semi-quantitative reverse transcription polymerase chain reaction (RT--PCR) and immunohistochemical technique. Results: Compared to sinus rhythm group, the expression levels of mRNA and protein of bFGF significantly increased in the paroxysmal AF group and chronic AF group (P〈0. 001 all), and the levels of mRNA and protein of bFGF significantly increased in chronic AF group compared with those of paroxysmal AF group (P〈0. 05 all). The mRNA and protein level of bFGF was positively correlated with AF duration (r=0. 330, P=0. 005 and r=0. 299, P=0. 010, respective) and left atrial dimension (r=0. 342, P=0. 003 and r=0. 285, P=0. 015, respective). Conclusion: The increased level of bFGF gene expression in atrium could be involved in the molecular mechanisms of atrial fibrosis during fibrillation, which might correlate with the initiation and maintenance of atrial fibrillation.
