机械通气对急性肺损伤大鼠炎症反应的影响 被引量:8

Influence of ventilation on inflammatory response in rat with acute lung injury

摘要 目的:评估不同潮气量对急性肺损伤(ALI)大鼠炎症反应的影响。方法:雄性SD大鼠30只,随机分为对照组、ALI组、小潮气量(LV)组、传统潮气量(NV)组及高潮气量(HV)组5组。机械通气4 h后,进行肺组织病理切片Smith评分、肺水含量(W/D)测定、肺通透性(LPI)测定(放免法测定血浆及肺泡灌洗液中125I-白蛋白含量,计算两者比值),ELISA法测定肺泡灌洗液中TNF-α、IL-10含量。结果:Smith评分、W/D LV组较ALI、NV、HV组显著减轻。肺泡灌洗液中TNF-α、IL-10含量LV组较NV、HV组显著减低。结论:小潮气量机械通气能减轻肺部炎症反应,从而减轻肺组织损伤,对ALI具有保护作用。 Objective To evaluate the influence of ventilation with different tidal volume on inflammatory response in rats with acute lung injury(ALI). Methods 30 male SD rats were randomly divided into five groups: normal group, ALI group,low tidal volume(LV) group, traditional tidal volume(NV) group,high tidal volume(HV) group. After 4 hours of mechanical ventilation, lung injury was evaluated by Smith lung injury score, lung water content( wet/dry, W/ D ) and lung permeability index (LPI) , tumor necrosis factor- α ( TNF- α) and interleukin- 10 ( IL- 10 ) levels in bronchoalveolar lavagc fluid (BALF) were measured by ELISA. Results Smith lung injury score and W/D in LV group were significantly lower than those in NV group and HV group, and Smith lung injury score, W/D, and LPI in HV group was the highest of all groups. Concentration of TNF-α and IL-10 in LV group were significantly lower than those in NV group and HV group. Conclusion LV ventilation can inhibit inflammatory response and alleviate lung injury in rat with acute lung injury.
