摘要 目的评价肌钙蛋白(cardiac troponin-I,CTn-I)与肌酸激酶(CK)、肌酸激酶同工酶MB(creatine kinase isoenzyme MB,CK-MB)等检测指标对急性心肌梗死的诊断的临床准确性。方法对近二年所测定的150例患者的心肌酶谱及肌钙蛋白(CTn-I)和临床诊断治疗的资料做了回顾性分析。结果通过以上分析,我们可以看出CTn-I检测兼有心肌酶的优点,同时比心肌酶特异性更高,窗口期更长、阳性结果表现更早,金标法CTn-I检测卡操作更为方便,尤其适合急诊床边检测的需要。以往通过心电图和心肌酶进行确诊时容易出现漏诊或操作繁琐,往往延误了治疗的最佳时机,而CTn-I检测卡可以用全血在15min之内快速诊断,可以在第一时间为临床治疗提供最为准确的诊断依据。结论CTn-I能提高诊断的准确率和减少漏诊。 Objective To compare accuracy in the diagnosis of acute myocardial infarction with cardiac troponin-I(CTn-I),creatine kinase (CK) and creatine kinase isoenzyme MB (CK-MB). Methods A retrospective analysis was made of myocardial enzymes and CTn-I in 150 cases of suspected acute myocardial infarction. Results The CTn-I detection in the diagnosis of acute myocardial infarction not only had the advantages of myocardial enzymes but also had its own advantages of higher specificity, a longer window period, earlier appearing of positive results,more convenient operation of the standard CTn-I Card detection,particularly suitable for the needs of bedside emergency,whole blood rapid diagnosis by CTn-I Card detection within 15 min and the first time to provision of the most accurate clinical diagnosis as earlier as possi- ble. Conclusion CTn-I can improve the diagnostic accuracy and reduce misdiagnosis.
