摘要
目的:应用血管回声跟踪(echo-tracking,ET)技术检测无颈动脉硬化超声诊断指征高血压合并高脂血症及糖尿病患者颈动脉血管弹性及功能。方法:筛选高血压合并高脂血症及糖尿病患者(三高组)30例,高血压患者20例,高脂血症患者20例,糖尿病患者20例,正常对照组30例及国人正常值,应用ET技术测量僵硬度(β)、压力应变弹性系数(Ep)及顺应性(AC),脉搏波速度(PWVβ),增大指数(AI),所得参数间进行比较。结果:三高组与正常组相比,三高患者β、Ep、PWVβ、AI较正常组明显增高,P<0.01;AC较正常组降低,P<0.01。三高组与国人组相比,β、Ep、PWVβ、AI明显增高,P<0.01,AC较国人组降低,P<0.01。三高组与高血压组、高脂血症组及高血糖组相比,β、Ep增高,P<0.01;PWVβ、AI增高,P<0.05;AC较各组降低,P<0.01。结论:①研究表明:三高患者颈动脉β、Eρ、AI及PWVβ较对照组明显升高;AC明显降低,各项参考值的改变说明三高患者动脉弹性、顺应性下降,硬化程度增加。②应用ET技术在超声确诊颈动脉硬化之前准确定量评价三高患者颈动脉弹性功能改变,旨在寻找动脉硬化的早期预测指标,为临床早期预防大动脉病变提供依据。
Objective:Using e-tracking(ET) technique to evaluate the carotid elasticity quantitatively in grouping patients of hypertension and hyperlipidemia and diabetes without atherosclerosis.Methods: 30 cases with hypertension and hyperlipidemia and diabetes;20 cases with hypertension and 20 cases with hyperlipidemia and 20 cases with diabetes.Another 30 healthy cases were selected and the normal standards of Chinese people as normal control.With ET technique,we calculated the carotid elasticity indexes: stiffness parameter(β),strain elastic modulus(Ep),arterial compliance(AC),one point pulse wave velocity(PWVβ),augment index(AI).Results: Compared with the normal cases,the data of β and Ep and PWVβ and AI were significantly higher in hypertension and hyperlipidemia and diabetes patients,P〈0.01;the data of AC was significantly lower in hyperlipidemia patients,P〈0.01.Compared with the normal cases,Compared with the normal standards of Chinese people,the data of β and Ep and PWVβ and AI were significantly higher in hypertension and hyperlipidemia and hyperglycemia patients,P〈0.01;the data of AC was significantly lower in hypertension and hyperlipidemia and hyperglycemia patients,P〈0.01.Compared with the hypertension cases and hyperlipidemia cases and hyperglycemia cases,the data of β and Ep were significantly higher in hypertension and hyperlipidemia and hyperglycemia cases,P〈0.01,the data of PWVβ and AI were higher in hypertension and hyperlipidemia and hyperglycemia cases,P〈0.05;the data of AC was lower in hypertension and hyperlipidemia and hyperglycemia patients,P〈0.05.Conclusion:①verpass the study:β、Ep、AI and PWVβ were significantly higher in hypertension and hyperlipidemia and hyperglycemia patients,than that in the control group.AC was lower than that in the control group.Elastic and arterial compliance were lower and stiffness was higher in hypertension and hyperlipidemia and hyperglycemia patients,blood vessel endothelium of hypertension and hyperlipidemia and hyperglycemia patients has been damaged.②ET can quantitative evaluate carotid endothelium function of the patients with hypertension and hyperlipidemia and diabetes.And it may be helpful to prevent great arteries diseases.
