颈部冰敷在腹腔镜甲状腺术后的临床应用

Clinical ap plication of neck ice compress on following laparoscopic thyroidectomy operation

目的:探讨腹腔镜下甲状腺术后使用颈部冰敷是否能够预防或减少术后并发症及减轻伤口疼痛。方法:将2007年10月～2009年10月行腹腔镜下甲状腺术后的患者60例随机分为研究组和对照组,各30例。研究组给予颈部冰敷,对照组未颈部冰敷,只给予常规护理。观察两组患者术后的伤口疼痛程度、颈部肿胀、引流量及拔管时间等。结果:研究组与对照组比较,给予颈部间断颈部冰敷后可以显著减轻术后伤口疼痛和减少伤口出血(P<0.001);预防或减轻颈部肿胀在术后24h内与对照组相比,有显著性差异(P<0.01),术后48h时无显著性差异(P>0.05)。结论:腹腔镜下甲状腺术后使用颈部冰敷可以预防或减少术后并发症的发生,减轻伤口疼痛,从而减轻患者的痛苦,且经济、简单易行,值得临床推广应用。

Objective： To dicuss whether apphng neck ice compress on following laparoscopic thyroidectomy operation can prevent or reduce postoperative complications and relieve pain. Methods： 60 cases of patients with laparoseopic thyroideetomy operation from October 2007 to October 2009 were divided into two groups （research group and control group）, each 30 cases. The research group was given the neck ice compress, but only routine care in the control group. Degree of postoperative wound pain, neck swelling, drainage and the time of removing drainage tube were observed between the two groups. Results： Compared with control group, postoperative wound pain and bleeding were significantly relieved in the research group （P〈0.001）, to prevent and relieve neck swelling was a significant difference （P〈0.01） within 24 hours, but no significant difference within 48 hours （P〉0.05）. Conclusion： Appling neck ice compress on the patients with laparoscopic thyroidectomy can decrease postoperative complications, relieve wound pain. It is feasible and economical and worthy of spreading in clinical use.
