鼻石诊断与治疗(附8例报告)

Diagnosis and management of rhinolith(with a report of 8 cases)

目的目的探讨鼻石的常规诊断与治疗程序。方法回顾性分析8例鼻石患者的临床资料,总结其诊疗经验与教训。结果本病患者的主要症状为鼻塞和流脓涕;高分辨CT扫描(HRcT)于6个病灶中心见低密度核心区域,周围为均匀的高密度无机盐影,是为本病较特征性影像学改变,并可发现并发的鼻窦炎症和中隔偏曲或穿孔。本组所有患者均行鼻内镜手术治疗。结论HRCT有助于鼻石的诊断和鉴别诊断,本病的适宜疗法为鼻内镜下手术治疗。

Objective To sum up clinical experience in the routine procedures for the diagnosis and management ofrhinolith based on a clinical trial. Methods A retrospective study was carried out among 8 cases with rhinolith treated in our Hospital to sum up the clinical experience in the diagnosis and management of such a lesion, with associated literatures reviewed at the same time. Results It could be summed up that the critical clinical complains of cases with the lesion of rhinolith included purulent discharge and nasal obstruction. On the films of high resolution CT （HRCT） scaning, six cases showed a lower density core area in the central region of the focus as a special feature, with higher density of peripheral area around the core suggesting deposition of mineral salts in this area. This feature should be the characterristic image finding on CT films. Moreover, demonstrated also could be the complications from the lesion, such as maxillary sinusitis and deviation or perforation of nasal septum. All these patients in this group studied here were treated by nasal endoscopic surgery （NES）. Conclusions Evaluation on the image features on HRCT films can aid the diagnosis and differential diagnosis of rhinolith and NES should be the optimal therapy for the treatment of such a lesion.
