小瞳孔白内障超声乳化术探讨

Phacoemulsification in microcoria cataract

目的:探讨小瞳孔状态下白内障超声乳化术的手术技巧。方法:临床确诊的小瞳孔白内障患者60例68眼,术中不切开瞳孔缘,采用"T"型调位钩牵引扩张瞳孔,原位进行超声乳化手术。结果:术后55眼(81%)瞳孔圆形,直径3～4mm;13眼(19%)瞳孔欠规整,直径4～5mm,其中9眼(13%)虹膜不同程度损伤;术后视力均提高,裸眼视力≥0.8者20眼(29%),0.5～0.8者25眼(37%),0.3～0.5者15眼(22%),0.1～0.3者8眼(12%);无严重并发症发生。结论:利用"T"型调位钩牵引扩张瞳孔,小瞳孔白内障的超声乳化手术可顺利进行。

AIM： To explore the surgery techniques of phacoemulsification in microcoria cataract. METHODS： Sixty cases （68 eyes） of microcoria cataract were firstly operated by using“T”-type positioning hook pupilloplasty, then in situ phacoemulsification were carried out,in which the pupil edge was not cut. RESULTS： The postoperative pupils recovered normal in 55 eyes （81%） , which got a diameter of 3-4mm; 13 eyes （19%） got a diameter of 4-5mm with abnormal pupils, of which 9 eyes （13%） got various iridic injury; the postoperative visual acuity was improved in 68 eyes （100%）, in which it was 0.8 or higher in 20 eyes （29%）, between 0.5 and 0.8 in 25 eyes （37%）, between 0.3 and 0.5 in 15 eyes （22%）, between 0.1 and 0.3 in 8 eyes （12%）; there was no serious complications.CONCLUSION： Using “T”-type positioning hook pupilloplasty, phacoemulsification in microcoria cataract can be carried out successfully.
