早期气管切开治疗高位颈髓损伤合并呼衰的应用价值

The Practical Value of Applying Tracheotomy to Respiratory Failure Caused by Upper Cervical Injuries in the Early Phase

目的:探讨气管切开在治疗高位颈髓损伤合并呼衰病例中的应用价值。方法:回顾性分析我院1995年1月至2005年5月期间收治的高位颈髓损伤患者共215例,其中117例合并呼吸衰竭并气管切开治疗。对照分析比较气管切开前后血氧分压、血二氧化碳分压、血WBC、血氧饱和度的变化。结果:高位颈髓损伤患者早期气管切开可有效地解决了通气和换气障碍(P<0.05),和控制感染(P<0.05)。结论:早期气管切开治疗高位颈髓损伤合并呼衰,可充分解决排痰、呼吸麻痹、感染等问题,提高生存率。

Objective： To discuss the practical values that tracheotomy used in curing respiratory failure caused by upper cervical injuries. Method： In the retro-spective study of 215 cases of upper cervical injury patients in our hospital during January in 1995 and May in 2005, the changes of partial pressure of oxygen （PO2 ） , partial pressure of carbon dioxide （ PCO2 ）, blood WBC and blood oxygen saturation before and after tracheotomy in 117 cases with respiratory failure patients received tracheotomy were compared and analyzed. Results： The upper cervical injury patients who received tracheotomy in the early phase can be solved the problem of breathing （ P 〈 0. 05 ） and control the contraction （P 〈 0. 05 ）. Conclusion ： Applying tracheotomy to upper cervical injury patients in the early phase can resolve sputum, respiratory failure and contraction, and improves the survival rate.
