机械通气治疗急性左心衰竭的护理

Nursing of Mechanical Ventilation in the Treatment of Acute Left Ventricular Failure

目的探讨机械通气治疗急性左心衰竭的疗效。方法对36例急性左心衰竭病人采用无创面(鼻)罩正压通气(Bi-PAP),6例失败者给予气管插管机械通气治疗,全部病例同时给予常规药物治疗。并观察通气前、通气后20分钟、1个小时患者的临床症状变化、心率、呼吸频率、血压及1小时血气分析。结果36例中30例采用BiPAP治疗成功,有6例改用气管插管机械通气治疗,也全部成功。结论机械通气治疗急性左心衰竭不仅见效快,且抢救成功率高,值得临床推广应用。

Objective To explore the curative effect of mechanical ventilation in the treatment of acute left ventricular failure. Methods 36 cases of patients with acute left ventricular failure were treated with noninvasive bilevel positive air pressure （BiPAP） ventilation, 6 cases of failures with treatment of intubation and mechanical ventilation. All patients received conventional drug treatment simultaneously, to observe the changes in patients with clinical symptoms 20 minutes, 1 hour before and after ventilation, heart rate, respiratory rate, blood pres sure and 1 hour blood gas analysis. Results In 36 cases, 30 cases were successfully treated with BiPAP in all indexes as well as another 6 failures. Conclusions Mechanical ventilation in the treatment of acute left ventricular failure has advantages of quick effects and high rate of successful rescue, which deserves popularizing in clinic.
