经单侧额下入路切除巨大前颅底脑膜瘤

Microsurgery through unilateral sub-frontal approach for huge meningioma at anterior skull base

目的总结经单侧额下入路切除巨大前颅底脑膜瘤的经验。方法报告6例巨大前颅底脑膜瘤,采用经右侧单侧额下入路手术,介绍体位、肿瘤显露和处理技巧。结果 6例肿瘤均显露良好,Simpson Ⅱ级切除,其中4例症状明显改善。结论对于巨大前颅底脑膜瘤,单侧额下入路较为便捷,创伤小,值得作为一种手术方式推荐给有丰富显微外科技术的医师。

Objective To summarize the experiences for resection of huge meningiomas at anterior skull base through unilateral sub-frontal approach. Method 6 patients with huge meningioma at anterior skull base removed through unilateral （right） sub-frontal approach were reported and body posture, tumor exposure and surgical technique were introduced. Results All the tumors of 6 eases were satisfactorily exposed and removed by Simpson grade Ⅱ. Symptoms of 4 eases were significantly improved. Conclusion Unilateral sub-frontal approaeh is much simple and convenient for resection of huge meningioma at anterior skull base. This approach is associated with less injury than the traditional accesses, so it could be worthy of recommending to the microsurgical surgeons with experienced hands.
