椎管内肠源性囊肿

Intraspinal enterogenous cysts

目的探讨椎管内肠源性囊肿的临床特点和外科治疗。方法回顾性分析2004年1月～2009年1月收治的12例椎管内肠源性囊肿患者的临床表现、影像学特征及治疗效果。本组男8例,女4例。囊肿位于颈段6例,胸段3例,腰段3例。结果全部病例均行显微手术治疗,术中全切8例,次全切2例,部分切除2例。术后除1例患者早期肌力下降外其余患者神经功能均明显改善。随访6月～5年(平均18个月),8例全切者无复发,4例次全或部分切除者2例复发。结论椎管内肠源性囊肿为先天性良性疾病,早期显微手术切除可获得良好效果。

Objective To explore the clinical features and surgical treatment of intraspinal enterogenous cysts. Methods The clinical presentation,image features and the therapeutic outcomes of 12 patients with intraspinal enterogenous cysts in our department from January 2005 to January 2009 were analyzed retrospectively. Of 12 patients, there were 8 males and 4 females. The cysts located in the cervical region in 6, thoracic region in 3 and lumbar region in 3. Results Of 12 patients treated by microsurgical operation, total removal was achieved in 8 （67%）, subtotal removal in 2 （17%）and partial removal in 2 （17%）. There was significant improvement in neurological function in all patients except that one patient aggravated in his motor deficit in the early stage after operation. All patients were followed up from 6 months to 5 years with an average of 18 months. No recurrence occurred in the 8 underwent total excision. Of the 4 patients who underwent subtotal or partial resection, recurrence occurred in 2 patients. Conclusion Intraspinal enterogenous cyst is the congenital benign disease. Early microsurgical resection can get excellent or good result.
