2型糖尿病一级亲属中餐后1小时血糖升高者胰岛素抵抗及分泌功能的研究 被引量:4

摘要 目的研究2型糖尿病一级亲属中口服葡萄糖耐量实验中餐后1h血糖升高者胰岛素抵抗和早期相胰岛素分泌功能.方法111例受试者分为4组,分别为非糖尿病一级亲属正常糖耐量组;糖尿病一级亲属正常糖耐量组;糖尿病一级亲属IGT组;糖尿病一级亲属餐后1h血糖高组,进行75gOGTT实验和胰岛素释放实验.由稳态模型(HOMA)法计算HOMA-IR评估胰岛素抵抗,由△I30/△G30评价早时相胰岛素分泌功能.结果与正常对照,糖尿病一级亲属正常糖耐量组相比,HOMA-IR在糖尿病一级亲属IGT组;糖尿病一级亲属餐后1h血糖高组明显升高,后两组间无统计学意义;△I30/△G30在糖尿病一级亲属IGT组;糖尿病一级亲属餐后1h血糖高组减低,而在后两组间差异无统计学意义.结论2型糖尿病一级亲属中糖负荷餐后1h血糖升高者其胰岛素抵抗程度和早期相胰岛素分泌功能与糖尿病一级亲属IGT人群相似,均存在显著的胰岛素抵抗和早期相胰岛素分泌功能减退. Objective To investigate insulin resistance and insulin secretion in the first degree relatives of 2 diabetics with hyperglycemia at 60'during OGTT.Methods A total of 111 subjects,including lean controls(NC),the first degree of 2 diabetics with normal glucose tolerance(NGT),the first degree of 2 diabetics with post-loading hyperglycemia(PLH)and the first degree of 2 diabetics with impaired glucose tolerance(IGT),underwent 75 g OGTT and insulin releasing test.Insulin resistance was determined by HOMA-IR.Insulin secretion was determined by △I30/△G30.Results Compared with NC and NGT,HOMA-IR was significantly increased in groups of PLH and IGT and there was no significant difference between the latter two groups,△I30/△G30 was significantly lower in PLH and IGT groups than in NC and NGT groups,and there was no significant difference between the former two groups.Conclusions The first degree relatives of type 2 diabetics with hyperglycemia at 60' during OGTT have a similar degree of insulin resistance and dysfunction of cell to those first degree relatives of type 2 diabetics with impaired glucose tolerance.
