上海市三级医院日门诊量"撑不下",大量二级和社区医疗机构却"吃不饱",医疗资源"过度使用"与"严重浪费"并存。为了提高医疗资源使用效率,上海市探索构建区域性医疗卫生联合体。1.医疗联合体采用何种治理结构,将直接关系到医疗卫生资源的使用效率。2.医疗联合体的管理体制需要政府进行宏观调控,实行法人治理结构。3.医疗联合体对住院医师的基础培养要建立完善的规划体系,使各级医疗机构的住院医师在学术和技术上同步发展。4.医疗联合体的信息化管理使各级医疗机构诊疗信息和健康档案信息整合并共享。

In Shanghai, the daily outpatient number in third level hospitals is extremely great while it is little in lots of second-level hospitals and community medical institutions, which leads to overspend and severe waste of health resources. The regional medical consortium will be established in Shanghai to increase the efficiency in the use of health resources. 1. The administrative framework of medical association should be related with the efficiency in the use of health resources. 2. The administrative system of medical consortium should be regulated by the government and managed by juridical person. 3. The basic train system for resident physicians should be improved in plan, so that the resident physicians in different levels of medical institutions could make progress together in science and technology. 4. The diagnostic and health record information should be conformed and shared in all levels of medical institutions.
