全直肠系膜切除联合双钉吻合行低位直肠癌保肛手术26例报告

Anus preservation operation for low rectal cancer by using DST combined with TME(a report of 26 cases)

目的探讨全直肠系膜切除联合双钉吻合在低位直肠癌保肛手术适应证,根治安全的可行性及注意事项。方法采用TME联合双钉吻合行低位直肠癌切除26例。结果全组均痊愈出院,其中出现术后并发症,吻合口漏2例,吻合口狭窄2例,切口感染2例,局部复发2例。随访36个月内死亡2例。远期排控便功能良好。结论TME联合双钉吻合行低位直肠癌保肛手术,安全方便,可达到根治与功能保全的双重效果。

Objective To investigate the surgery indication, security and feasibility effects with radical correction, and cautions of double stapling technigne combined with total mesorectal excision for anus preservation operation on low rectal cancer. Methods Twenty - six patients of low rectal cancer were operated by using DST combined with TME. Results All cases had succesful recovery. The anastomotic leakage occurred in 2 cases, and anastomotic stricture occurred in 2 cases, and incision infection occurred in 1 case, local recurrence occurred in 2 cases in this group. All patients were followed up for 36 months and death ocureed in 2 patients The long - term function of exhaust and control stool is good. Conclusion It is secure and convenient by using DST combined with TME for anus preservation operation on low rectal cancer which can reach double effects of radical correction and function preserved.
