摘要 目的研究人体跟腱病变的磁共振成像(MRI)的临床实用价值。方法应用磁共振成像分析14例有跟腱病变病例,归纳特征性的MRI表现。结果根据MRI图像诊断跟腱完全撕裂6例,磁共振特征性表现为跟腱肌腱不规则增粗,跟腱肌腱束连续性完全中断,跟腱肌腱断端交错改变,在T_1WI、T_2WI上其内呈混杂信号;部分撕裂8例,MRI表现为跟腱肌腰增粗、宽径/前后径比值变小,在T_1WI、T_2WI上肌腱内局限性高信号,肌腱束部分毛糙、不规则,在矢状位至少有1个层面是连续的;跟腱外伤所致炎性感染2例,MRI表现为肌腱增粗,边缘毛糙、模糊不清,周围软组织肿胀。手术或临床资料综合分析结果:完全撕裂8例,部分撕裂6例,外伤所致炎性感染1例。结论磁共振成像能清晰显示跟腱病变,完全可以准确评估损伤撕裂的位置及程度,可以对临床手术治疗提供准确的依据并具有较好的指导作用。 Objective To investigate the diagnosfic value of achilles tendon laceration by MR images. Methods Delineated MRI findings in 14 patients who had history of ankle joint trauma and were proved the complete or partial achilles tendon laceration were analyzed.The characteristic manifestations of MR were summarized. Results The achilles tendon laceration was diagnosed on MRI in all cases, according to the differential MRI features, complete achilles tendon rupture was shown in 6 cases and partial lacera- tion in other 8 cases; The diagnosis incluede complete achilles tendon rupture 8 cases and partial rupture 6 cases by comprehensively analyzing surgical or clinical fmdings. Con- clusion MRI can clearly show the structure of achilles tendon, and accurately diagnose the portion and the degree of achilles tendon laceration, which provide evidence and guidance for surgical treatment.
