磁共振影像在脑静脉和静脉窦血栓诊断中的价值

Value of MRI Imaging in the diagnosis of cerebral veins and sinuses thrombosis

目的探讨MRI和MRV对脑静脉和静脉窦血栓(CVST)形成的诊断价值。方法回顾性分析16例CVST患者的临床和影像学资料。结果多为中青年,急性或亚急性起病,临床表现为头痛、呕吐、伴或不伴局灶神经功能缺损症状或癫痫发作,影像学表现为静脉窦闭塞及静脉性梗死及脑出血灶,MRV表现为受累静脉窦部分或广泛高血流信号的缺失、狭窄。应用脱水、抗凝、溶栓治疗。16例中13例完全恢复正常,3例死亡。结论MRI和MRV是诊断和随访CVST形成的最佳检查方法,对脑静脉、静脉窦血栓的诊断优于CT,可替代创伤性的DSA检查。

Objective To investigate the diagnosis value of MRIand MRV for the cerebral venous and sinus thrombosis（CVST）. Methods The clinical and imaging data were retrospectively analyzed in 16 cases of CVST. Results The CVST occurred in the middle age and young adults mainly.Most got the illness acutely or subacutely,The early clincal characteristics were headache,vomit some accompanied with focal neuro- logic impairment or seizure.The imaging finding in CVST were venous sinus occlusion, venous infarction and hemorrhage, The signs of dural sinus thrombosis on MRV included the lack of a typical hihg-fiow signal from a sinus and stenosis.ALL of them were treated with dehydration,anticoagulation and throbolysis. 13 of 16patients recovered.3 of 16 patients died. Conclusion MRIand MRV are the best choice in diagnosis and followup of CVST,superior to CT,and can replace invasive DSA.
