超声刀在开腹阑尾切除术中的应用价值

摘要 目的探讨超声刀在开腹阑尾切除术中的应用价值。方法将2007年7月~2009年7月我科收治的急性阑尾炎116例分成两组:术中使用超声刀的58例为观察组,术中未使用超声刀的58例为对照组。比较两组治疗效果。结果两组均顺利完成手术。在手术时间,肛门排气时间,住院天数方面,观察组均优于对照组,但在非坏疽穿孔性阑尾炎二者无统计学差异,而在坏疽穿孔性阑尾炎二者有显著性差异;两组住院费用比较无统计学差异;对照组切口感染率高于观察组,在急性坏疽穿孔性阑尾炎中尤为明显。结论超声刀用于开腹阑尾切除术具有手术时间短,术后恢复快,住院时间缩短,切口感染率低的优点,在坏疽穿孔性阑尾炎中尤为明显。而住院总费用并未增加,安全实用。 Objective To discuss the feasibility of the use of ultrasonic scalpel in appendectomy. Methods 116 acute appendicitis patients were divided into 2 groups. In research group, 58 patients received appendectomy and were applied ultrasonic scalpel during operation. In control group, 58 patients received appendectomy by using classical technique. All patients came from our hospital from Jul. 2007 to Jul. 2009. Results All operations were successfully performed. There was no significant difference between the two groups in the operative time, the postoperative mean time of aerofluxus, the hospital stay and the rate of infection of incisional wound. However, strong difference was observed in the gangrened and perforated appendicitis. The hospitalization expenses between the rearch group and the control group were not significantly different. Conclusion The usage of ultrasonic scalpel in appendectomy of acute appendicitis is safe and feasible, which has the practical values of shortening the operating time, reducing the hospital stay time, reducing the rate of infection of incisional wound and promoting recovery, especially to gangrened and perforated appendicitis. The technique is safe and economic.
