下眶睑区凹陷型睑袋的整形术

Plasty of depressed baggy deformity of lower eyelid

目的探讨下眶睑区凹陷型睑袋的整复术式。方法在局部麻醉下释放出眶隔脂肪，对于单纯的下睑板下缘凹陷者，采用上提眶隔脂肪和折叠眼轮匝肌的方法修复；对于下眶缘及眶鼻沟凹陷者，将眶隔脂肪下移到凹陷区修复；对于下眶缘、眶鼻沟和下睑板下缘均有凹陷者，将眶隔脂肪水平劈成上下2片分别上提和下移至凹陷处修复。结果20例患者术后获随访7d至7个月，凹陷得到良好矫正，眶区外观饱满有张力，给人以年轻化之感，无睑缘外翻，局部臃肿等并发症发生。结论针对凹陷出现的不同部位，采用相应的方法矫正可以取得良好的美容效果。

Objective To explore the plastic surgical procedures for depressed baggy deformity of lower eyelid. Methods With local anesthesia, intraorbital septum fat was released. For the eye bags with simple depression in region palpebralis inferior, the released fat was transferred upward combined with reflexed sphincter oculi to fill in the depressed region; for depression both in inferior orbital margin and orbitonasal furrow, released fat was transferred downward to fill in the depressed region; for depression in inferior orbital margin, orbitonasal furrow and inferior margin of tarsus inferior palpebrae, the fat was split into two parts, one was transferred upward combined with reflexed sphincter oculi to fill in the depressed in inferior margin of tarsus inferior palpebrae, another was fill in the depressed regions of inferior orbital margin and orbitonasal furrow. Results After 7 days to 7 months follow-up, all 20 cases in this group achieved satisfactory correction in the depressed regions with rejuvenational appearances and without ectropion of eyelid or overcorrection. The tension of autorbital wall was increased. Conclusion In regard to eye bag with different depressed regions, satisfactory effects can be achieved via suitable methods.
