夹板外固定结合中药治疗对老年桡骨远端骨折患者骨折愈合和腕关节功能恢复的影响

Effects of Controlled Trial on Sprint External Fixation Combined with Chinese Herbs on Fracture Healing and Wrist Joint Function Recovery in Elderly Patients with Osteoporotic Distal Radius Fracture

目的探讨夹板外固定结合中药治疗对老年人桡骨远端骨折患者骨折愈合及腕关节功能的影响。方法选择我院骨科84例原发性骨质疏松性桡骨远端骨折患者,手法复位小夹板外固定后,随机将患者分为中药组、西药组及对照组,每组28例。三组患者于治疗后第4、6、8、12周后摄腕部正侧位X线片,了解骨折愈合情况并进行对比,2个月后进行腕关节功能评估并进行疗效对比。结果中、西药组骨折愈合时间与对照组比较,差异有统计学意义(F=2.49、2.19,P<0.05),而中、西药2组比较差异亦有统计学意义(F=3.07,P<0.05)。中、西药组在腕关节功能恢复方面优良率均高于对照组(χ2=9.86、7.54,P<0.05),且经Ridit分析中、西药组与对照组疗效比较差异有统计学意义(P<0.05),中药组与西药组比较差异有统计学意义(P<0.05)。结论强骨活血汤方能明显促进骨折愈合,缩短骨折愈合时间,且优于西药,同时能显著促进腕关节功能的恢复。

Objective To explore the effect of controlled trial on sprint external fixation combined with Chinese herbs on the fracture healing and wrist joint function recovery in elderly osteoporotic distal radius fracture （ODRF） patients. Methods Eighty- four patients with ODRF were randomly divided into Chinese herbs group, western medicine group, and control group （each n= 28） after the fractures were fixed manually by splint. X- ray examination on the fractured wrist was taken at the 4, 6, 8, 12 weeks after treating, the fracture healing was observed, wrist function evaluation and comparison of curative effect were taken after two month treatment. Results There were statistically significant differences in the fracture healing time between Chinese herbs group and western medicine group （F = 3.07, P〈0.05）, and the fracture healing time in the control group was longer than that in Chinese herbs group and western medicine group （F=2.49, F=2.19, P〈0.05）. According to Dienst wrist rating system, the excellent and geed rates of the three groups showed statistically significant differences between the medicine groups and control group （Χ^2 = 9.86, Χ^2 = 7.54, P〈0.05）, and the curative effect of the medicine groups was more excellent than that of control group through Ridit analysis. Conclusions Qianggu Huoxue decoction can significantly promote the fracture healing and the wrist function recovery, and shorten the healing time, moreover, its curative effect surpasses western medicine.
