妊娠急性脂肪肝的临床分析

Clinical Analysis on Acute Fatty Liver of Pregnancy

目的探讨妊娠急性脂肪肝的诊断和治疗效果。方法回顾性分析我院2例妊娠急性脂肪肝患者的临床表现、实验室检查、妊娠结局并结合近年来文献报道进行总结。结果患者均以孕晚期纳差、腹胀、浮肿、黄疸、DIC为主要表现。超声呈"亮肝"表现。CT显示肝实质密度衰减。结论妊娠急性脂肪肝多根据临床表现和实验室检查临床早期诊断,尽早终止妊娠及支持对症治疗有望提高妊娠急性脂肪肝的母儿生存率。

Objective To discuss the diagnosis and therapy of acute fatty liver of pregnancy（AFLP）. Methods We made a retrospective study about two patients admitted in our hospital,analyzing their clinical situations,laboratory examinations and pregnancy outcomes,and provided a summary about AFLP based on literature and report during the past few years. Results The initial manifestations of the two patients included fatigue,anorexia,jaundice, edema in the third trimester of pregnancy. An abdominal ultrasound showed the light liver. Abdomen computed tomography（CT） showed a prominent fatty liver. Conclusion In most cases, early clinical diagnosis reliance on laboratory data and symptoms. Prompt delivery and supportive care provide significant improvement in outcome for both mother and bady.
