1H-MRS对老年甲状腺良恶性结节的临床鉴别 被引量:4

摘要 目的研究老年甲状腺良、恶性结节的氢质子波谱(1H-MRS)特点。方法本组共研究35例老年甲状腺结节,良性结节组16例,恶性肿瘤组19例,均行常规MRI扫描及氢质子波谱检查。在工作站上测量Cho/Cr比值并进行统计学分析。结果恶性结节组Cho/Cr比值明显高于良性结节组,差异均具有显著性意义(P<0.01)。与良性结节组相比,恶性肿瘤组织的Cho峰都明显升高,而Lip则明显下降。结论甲状腺良、恶性结节氢质子波谱表现明显不同,氢质子波谱对甲状腺结节的诊断与鉴别具有较高价值。 Objective To investigate the proton magnetic resonance spectroscopy ( 1H MRS) features of benign and malignant thyroid nodule in human. Methods 35 cases were included in the study in which there were 18 cases in benigntumor group and 18 cases in malignant tumor group respectively. The 1H MRS features of all 35 cases wereanalyzed and the correlation study between in vivo 1H MRS findings and that of in vitro 1H MRS was performed. Cho/Cr was calculated on workstation and a statistic test was done. Results The Cho/Cr value in the malignant tumor was larger than in the benign tumor, to benign tumor group, in vitro 1H MRS revealed increased Cho and decreased Lipid in 18 cases of the malignant tumor. Conclusions The benign and malignant thyroid nodule in human showed great difference in 1H MRS. In vivo 1H MRS findings demonstrated great consistencewith that of in vitro 1H MRS. In vivo 1H MRS has high application value in the differentiation diagnosis of thyroid nodule.
