摘要
目的应用血管回声跟踪技术(ET)评价缺血性脑血管病患者颈动脉弹性的变化,从而全面分析病因、评估预后。方法随机选择住院的短暂性脑缺血(TIA)患者30例、脑梗塞患者30例,正常对照组30例,应用ET术评估颈动脉硬化参数:血管压力应变弹性系数(Ep)、僵硬度(β)、顺应性(AC)、脉搏波传导速度(PWVβ)、增大指数(AI)、收缩期最大血管内径(Ds)及舒张期最小血管内径(Dd)。结果①脑梗塞患者β、Ep、AI及PWVβ较正常组升高,AC较正常组降低,且差异有统计学意义。TIA患者β、Ep、AI及PWVβ较正常组升高,AC较正常组降低,但差异无统计学意义。②脑梗塞组年龄<60岁者的Ds和Dd与≥60岁者无明显差异;正常组、TIA组年龄≥60岁者较<60岁者的Ds和DC增大(P<0.05)。结论ET技术能有效评价缺血性脑血管病患者颈动脉的弹性改变,为临床提供较丰富的信息。
Objective To investigate the changes of the elasticity of carotid artery in patients with ischemic cerebrovascular disease by echo tracking technology(ET) , and to analyze the etiological factors and evaluate the prognosis of the patients. Methods Thirty patients with TIA, 30 patients with cerebral infarction and 30 subjects as normal control group were enrolled in this study randomly. Then the parameters of carotid arterial sclerosis were evaluated by ET, including elasticity modulus (Ep) , artery stiffness ( β ) , artery compliance ( AC ) , pulse wave velocity ( PWVβ ) , augmentation index ( AI ) , blood vessel diameter of systolic phase(Ds) and blood vessel diameter of diastolic phase(Dd). Results The Ep, β, PWVβ and AI in cerebral infarction group were higher than those in normal group, but AC was lower than that in normal group, there was significant difference between two groups. The Ep, β, PWVβ and AI in TIA group were higher than those in normal group, but AC was lower than that in normal group, there was no significant difference between the two groups. There was no significant difference of Ds and Dd between age below 60 years and age above 60 years in cerebral infarction group, while the Ds and Dd in age above 60 years were higher than those in age below 60 years in the normal group and' the TIA group ( P 〈 0.05 ). Conclusion ET technology can effectively evaluate the elasticity of carotid artery changes in patients with ischemic cerebrovascular disease, and it can provide more information for clinical treatment and prevent the occurrence of cerebral infarction.
