纠衰汤治疗肺心病心衰60例的疗效观察

Observation on Therapeutic Effect of Jiushuai Decoction in Treating 60 Patients with Heart Failure due to Pulmonary Heart Disease

用益气活血、利水祛痰法组成的中药纠衰场和川芎嗪注射液静脉滴注治疗慢性肺原性心脏病急性发作期心衰病人60例，并与随机用西医综合治疗58例作为对照组。结果显示：中药治疗组总有效率为95％，明显优于西药对照组总有效率81．4％（P＜0．05）；治疗前后血气变化中药治疗组PaO_2、SaO_2、PaCO_2有极显著变化（P＜0．01），西医对照组则有显著变化（P＜0．05），治疗组优于对照组（P＜0．05）；治疗前后血液流变学变化中药治疗组全血粘度低切有极显著变化（P＜0．01），高切、全血还原粘度、RBC聚集指数、PLT粘附率有显著变化（P＜0．05），而西医对照组无明显变化。说明治疗肺心病心衰在西医一般疗法、控制感染的基础上用益气活血、利水祛疾的中药纠衰汤与川芎嗪静脉滴注能提高．疗效，值得推广应用。

60 patients with acute heart failure due to pulmonary heart disease were treated with Jiushuai decoction, which has effects of invigorating qi, activatingblood circulation, removing phlegm and promoting diuresis. Ligustrazine was jointly used by intravenous infusion. 58 cases treated with comprehesive therapy ofwestern medicine were taken as control group. The total effective rate of treated group was 95% and that of control group was 81. 4% (P < 0. 05). After treat-ment, PaO_2, SaO_2 and PaCO_2 were significantly changed as compared with before treatment (P<0. 01)in treated group,P<0. 05 in control group),the differ-ence between the two groups is significant(P<0. 05).In treated group . low shear rate and high shear rate of whole blood. reduction viscosity of whole blood,exythrocyte aggregation index and platelet adhesionrate were also significantly changed after treatment,inwhich low shear rate of whole blood was changed moremarkedly(P<0. 01), while the changes of these index-es were insignificant in control group. The results showthat combined use of Jiushuai decoction and ligus-trazine on the basis of general treatment of westernmedicine can raise the therapeutic effect of heart failuredue to pulmonary heart disease.
