改良根治术治疗乳腺癌2680例效果分析

Analysis of Treatment in Modified Radical Mastectomy of Breast Cancer(2680 Cases )

目的:探讨改良根治术治疗乳腺癌的临床疗效。方法:回顾性分析我科近14年来对2680例乳腺癌行改良根治术的治疗效果。结果:2680例中术后发生皮下积液462例,切口皮缘坏死35例,上肢淋巴性水肿(轻度)5例,胸肌挛缩伴同侧上臂运动障碍5例;局部复发19例,腋下复发26例,远处转移17例;3年生存率为97.7%,5年生存率92.6%,10年生存率82.1%(316/385)。结论:改良根治术可作为治疗乳腺癌的首选术式,可明显提高患者生存质量。术中保护胸部主要神经可获得良好的胸廓外形,保持肢体功能,术后给予综合治疗可提高远期疗效。

Objective ： To investigate the clinical efficacy of modified radical mastectomy of breast cancer. Methods： Data on the 2 680 cases of breast cancer treated with modified radical mastectomy during the past 14 years were retrospectively analyzed. Results ： Among 2 680 cases,there were 462 cases of seroma formation,35 cases of skin flay necrosis,12 cases of upper limb lymphoedema,5 cases of muscle contracture associated with ipsilateral upper limb dyskinesia;local recurrence occurred in 19 cases,axillary recurrence occurred in 26 cases,distant metastasis in 17 cases;three-year survival rate was 97.7% ; five-year survival rate was 92.6% ; ten-year survival rate was 82.1% （316/ 385）. Conclusion：Modified radical mastectomy for breast cancer can improve the patients＇life quality and should be used as the preferred method. Protection of the lateral thoracic never from upper limb dyskinesia can get better thoracic shape. Comprehensive treatment is crucial for bettter curative effect in the long term.
