住院精神分裂症患者合并法定传染病管理现状

Current Management Situation of Hospitalized Schizophrenia Case with Notifiable Infecticus Diseases

目的了解住院精神分裂症患者合并法定传染病的管理现状,为制定相关政策提供依据。方法采用问卷调查法对安徽省10家地市级精神病专科医院进行调查。结果2007年安徽省10家地市级精神病医院住院精神分裂症患者合并法定传染病上报人次184例,占住院精神分裂症患者总人数的1.37%。其中乙肝患者155例,占上报传染病总人数的84.24%;肺结核患者24例,占上报法定传染病总人数的13.04%。被调查的10家精神病专科医院中,多数未设置传染病区(80%);未设置传染病区的医院对精神病合并法定传染病患者在门诊排查后转院治疗有3家(30%),其余6家医院均采取就地隔离治疗的方式,严重患者请传染科会诊(60%)。精神科从业人员对传染病知识平均知晓率医生为84.6%,护士为84.1%。结论应加大对精神病专科医院在传染病防治方面基础建设与人力资源配置的投入,建立快捷有效的会诊转诊机制;对合并法定传染病的住院患者要严格执行消毒隔离措施,防止交叉感染。

Objective The paper analyzed current management situation of hospitalized schizophrenia with notifiable diseases and provided basis for working out relevant policy.Methods 10 mental disease hospitals in Anhui Province were investigated with questionnaire.Results 184 schizophrenic cases combined with notifiable diseases in the mental disease hospitals were reported in 2007,accounting for 1.369% of total schizophrenia cases.Of them,155 schizophrenia cases had hepatitis B,accounting for 84.24%.23 cases had pulmonary tuberculosis,accounting for 13.04%.Majority of 10 hospitals had not wards for infectious disease cases,accounting for 80%.30% hospitals transferred the cases to other hospitals.The rest hospitals isolated the cases in their own hospitals.The severe patients got consultation by infectious disease doctors,accounting for 60%.The average infectious disease knowledge rate was 84.6% for doctors,84.1% for nurses.Conclusion Local government should increase infectious disease doctors and wards in mental disease hospitals and establish effective transfer and consultation mechanism.In order to prevent transmission of the infectious diseases,strict isolation and disinfection measures should be taken.
