咪唑安定用于机械通气患者镇静治疗的临床观察

The clinical effect of midazolam on calm treatment for patients receiving mechanical ventilation

目的观察咪唑安定对机械通气患者镇静治疗的疗效。方法选择我科2007年6月-2008年5月住院患者中需机械通气的病例23例,首先缓慢静注咪唑安定5 mg的负荷量,然后持续静脉泵注维持量的咪唑安定0.04-0.2 mg·kg^-1·h^-1,以获得Ⅱ-Ⅲ级Ram say镇静评分,同时连续监测患者的SpO2、CVP及用药前和用药2 h后动脉血气及Ram say镇静分级。结果23例患者在负荷剂量下均能够达到镇静效果,Ram say评分均在Ⅲ-Ⅳ级;在维持剂量时也能保持等效的镇静效果,监测CVP平稳,血气指标及SpO2得到改善。结论咪唑安定是机械通气患者镇静治疗的有效药物。

Objective To study the effect of calming with midaxolam used for patients receiving mechanical ventilation.Methods Loading dose 5mg of midazolam was given by slow intravenous injection first.Ⅱ-Ⅲ level Ramsay calm gradings was obtained by continuous giving maintenance does 0.04-0.2 mg·kg^-1·h^-1 with transfer pump.Simultaneously continuous monitoring was given to patient＇s SpO 2 and CVP.Artery courage vigor and Ramsay grading were analyzed before the medication was applied and two hours after the medication.Results All patients could achieve the calm effect completely under the loading dose.The Ramsay grading was kept at Ⅲ-Ⅳ level.When maintenance dose could also maintain the equivalent calm effect and monitoring of CVP was steady,the courage vigor value and SpO 2 were improved.Conclusion Midazolam is effective on calm treatment for patients receiving mechanical ventilation.
