VCT在肝脏灌注异常诊断中的价值

The value of VCT in the diagnosis of hepatic perfusion disorders

目的运用VCT显示肝脏多种灌注异常,评价其在灌注异常诊断中的价值。方法回顾性分析3年中接受肝脏VCT平扫+增强扫描中出现灌注异常60例患者VCT图像资料,总结其影像表现及探讨发生机理。结果共发现各类异常灌注60例,根据其发生原因大致分为:肝动-门脉分流23例(38.3%);肝内、外血管阻塞16例(26.7%);炎症充血11例(18.3%);肿瘤盗血3例(5%);解剖变异、迷走血管供血3例(5%);无明确原因4例(6.7%)。结论VCT是一种准确评价肝脏灌注异常的有效检查方法。

Objective To display the various manifestations of hepatic perfusion disorders with VCT and evaluate the value of VCT in the diagnosis of hepatic perfusion disorders.Methods VCT findings of 60 patients with hepatic perfusion disorders in three years were retrospectively analyzed.The manifestations and mechanisms of different patterns of hepatic perfusion disorders were investigated.Results 60 cases were demonstrated to have hepatic perfusion disorders.The etiologies of various hepatic perfusion disorders were classified as arterio-portal shunts 23 cases（38.3%）,portal/hepatic venous obstruction 16 cases（26.7%）,inflammatory hyperemic change 11 cases（18.3%）,steal phenomenon by tumors 3 cases（5%）,anatom-variation and aberrant blood supply 3 cases（5%） and uncertain causes 4 cases（6.7%）.Conclusion VCT provides a new effective technique for demonstrating various hepatic perfusion.
