摘要 目的:探讨功能性显像技术在癫痫灶定位中的价值。方法:我们对60例癫痫分别进行脑正电子发射计算机断层(PET)、单光子发射计算机断层(SPECT)、MRI和CT检查,对癫痫灶进行定位。59例为PET发作间期显像,1例进行了发作间期和发作期显像,并与其CT和MRI结果进行比较,结果分析采用目测法。结果:发作间期阳性率PET为867%(52/60),SPECT为60%(36/60)。PET与SPECT定位完全符合者占806%(29/36)。PET定位癫痫灶的灵敏度明显优于SPECT、CT、MRI。结论:我们认为PET功能显像通过观察脑葡萄糖代谢进行癫痫灶定位是目前较好的无创性的检查方法。 PURPOSE:To evaluate the value of the functional imaging in localization of epileptic foci. METHODS:Positron emission tomograpy with 18 F-FDG、Single Photon Emission Tomograpy (SPECT)with 99m Tc-ECD、MRI、X-ray CT were performed on 60 epileptic patients. 59 patients were studied in the interictal stage,1 patient was performed in the ictal and interictal stage. The results were compared with anatomic images CT and MRI. The PET images were analyzed by visualization method. RESULTS:the sensitivity in interictal stage of PET、SPECT、CT、and MRI was 86 7% (52/60),60%(36/60),35%(21/60),50%(30/60)respectively, The completely correspond ratio of detecting epileptic foci by PET and SPECT was 80 6%(29/36),the sensitivity of PET was higher than SPECT、MRI、CT in detecting and localization epileptic foci. CONCLUSION:PET is more effective and noninvasive methods to localize epileptic foci, combination with anatomic imaging can offer more information for patients with epilepsy.
