冬眠合剂在小儿支气管异物取出术中的应用

Habination Mixture in Removal of Children's Bronchial Foreign Bodies under Local Anesthesia

目的探讨冬眠合剂在局部麻醉下小儿支气管异物取出术中的应用价值。方法小儿气管支气管异物患者21例随机分为对照组(n=10)和观察组(n=11),两组组均行局部麻醉下支气管异物取出术,观察组术前肌注冬眠合剂和阿托品,对照组使用苯巴比妥和阿托品。术中监测患者最快心率,最快呼吸频率等指标;术后观察患儿的行为,判断其心理状态。结果对照组患儿术中表现较强的挣扎,心率和呼吸较快,术后多有不安、哭闹,畏惧检查。观察组能较为平稳地接受检查及完成异物取出,患儿心率及呼吸频率明显少于对照组;两组术后心理状态比较,使用冬眠合剂患儿优于对照组患儿。统计分析显示上述差别具有统计学意义(P<0.05)。结论局部麻醉下小儿支气管异物取出术中应用冬眠合剂,可以减少术中损伤和心理创伤,避免恶性心理刺激。

Objective To explore the practical value of habination mixture in removal of bronchial foreign bodies in children un- der local anesthesia. Methods 21 children with tracheobronchial foreign body were randomly divided into a control group and an ob- servation group. All the cases removed the bronchial foreign bodies utilising local anesthesia, a pre - operative habination mixture and atropine were intramuscular injected in the control group, and phenobarbital atropine in the observation group. The maximum heart beat and breathing frequency were monitored during the operation. In addition, the behaviors of these children were observed, and their mental state evaluated. Results Children in the control group struggled during the operations, the heart beat rates and breathing fre- quencies were higher, and they were either uneasy, crying, or fearful of the post operative check - up. The cases in the observation group were stable during the operation, and the heartbeat rates and breathing frequencies were noticeably lower than the observation group. The comparison of psychological status in the two groups was also apparent. The control group was better than the observation group. Conclusions Habination mixture in removal of bronchial foreign bodies in children under local anesthesia can reduce the intraoperative injury and psychological trauma, and avoid the vicious psychological stimulation.
