重庆婴幼儿轮状病毒分子流行病学特征及相关临床资料分析 被引量:20

摘要 目的研究重庆地区婴幼儿轮状病毒的分子流行病学特征和相关临床资料分析。方法收集2008年10月至2009年2月在重庆儿童医院就诊的患非细菌性急性腹泻的婴幼儿大便和问卷。先用胶体金法检测轮状病毒抗原,并经RT-PCR(逆转录PCR)方法证实为轮状病毒,然后用巢式RT-PCR方法进行VP7(G)、VP4(P)基因分型并分析相关临床资料。结果共收集到507份大便标本,胶体金法检测轮状病毒抗原阳性率50.69%(257/507)。147份轮状病毒VP6基因阳性标本中G分型结果:G1型占40.14%(59/147);G2型6.80%(10/147);G3型17.01%(25/147);G4型2.04%(3/147);G9型0.68%(1/147);尚有49份未能分型,占33.33%(49/147)。P分型结果:P[8]型占60.54%(89/147);P[4]型占6.12%(9/147);P[9]型和P[10]型各有1例,分别占0.68%(1/147);尚有47份未能分型。G、P型别组合最常见的是G1/P[8]型,占58.93%(33/56)。各G、P型别间临床症状比较无统计学差异。结论G、P分型结果与其他地区明显不同,G分型显示出多样性特点,P分型仍以P[8]型占优势。 Objective To explore the molecular epidemiological characterization and analyze the related clinical data of rotavirus diarrhea among infants in Chongqing region. Methods Fecal specimens and questionnaires were collected from infants with non-bacterial acute diarrhea who were admitted at Children' s Hospital of Chongqing Medical University from October 2008 to February 2009. Nested reverse transcription polymerase chain reaction (nested RT-PCR) method was used for rotavirus-positive specimens which been verified by screening method with colloidal gold and proved by RT-PCR in order to analyze the VP7 and VP4 genotypes ( G- and P-typing). All the results and clinical questionnaires were analyzed. Results A total of 507 fecal specimens were collected. Rotavirus antigen positive rate was 50.69% (257/507). Out of the 147 rotaviruspositive specimens, Gl-type accounts for 40.14% (59/147), G2 6.80% (10/147), G3 17.01% (25/147), G4 2.04% (3/147) , and G9 0.68% ( 1/147), respectively. While 49 cases (33.33%) were failed to detect G-genotypes. Correspondingly, P[8] type accounted for 60.54% (89/147), P[4] 6.12% (9/147), P[9] and P[ 10] each of 0.68% ( 1/147), respectively, and 47 cases of failed P-genotyping. Conclusion Comparing with other areas, the constitutions of rotavirus G- and P-genotypes is different distinctly, and G-genotype shows the characterization of diversity in Chongqing region, however P[8]-genotype is still dominant. However, no significant different result is found when we compare different symptoms about acute diarrhea among almost every G and P-genotypes or G/P combinations.
